The Massachusetts Department of Transportation has replaced the manhole cover that flew off and killed a driver on I-93 last month. 

The new cover has been bolted down. 

The repair involved removing a rim from the manhole. 

That rim is now part of the investigation into the accident, which continues. 

An inspection of the highway system’s infrastructure, which started on Feb. 12, is complete, according to MassDOT.
