BOSTON (WHDH) -

Paperwork shows the driver involved in Monday's crash was involved in an accident in 2010.

Sources said following the accident four years ago, the man was fired but then re-hired by the MBTA.

Passengers were forced to walk the tracks after their Green Line trolley went off the tracks and smashed into a tunnel wall on Monday.

Sources tell 7News the employee driving that derailed trolley was fired back in 2010 for another accident that he did not report.

According to transit police paperwork: In September of 2010, there was an accident at the Park Street Station where a woman "had come into contact with a moving trolley."

The officer at the time wrote the woman, "admitted to bending over the yellow warning tape to retrieve an item," and when she did, "she failed to observe the moving inbound trolley."

Even though the report stated the driver was not at fault, sources said he was fired because he didn't report the accident.

Sources also said the union then filed a grievance and an arbitrator told the MBTA it had to give the driver his job back.

The operator's driving record revealed 20 moving violations, including seven for speeding, but the driver was hired in 2008, a year before the law requiring the MBTA and RMV to work together to screen new hires.

As for Monday's crash, investigators are still looking into what caused it, but MBTA officials said speed was a contributing factor.
